A guide for patients and their families.  
Problem/Why The Need For This Guide:
The lack of accessible and appropriate information for predialysis patients.

Purpose
The creation of an information guide for predialysis patients and their families which reflects the philosophy of the multi disciplinary treatment t and care the renal team were developing.

Design: The Process:
We began as a team looking at available literature for pre dialysis patients and constructed a framework of the information we thought would be most helpful for patients and their families. Each team member wrote a draft on the section they felt most qualified to write. The text was then put together in a common, easily understandable style by the co-ordination, p.d. nurse the psychologist. The illustrations were provided by a local artist. The first draft was circulated to all professionals involved and to renal patients and their families for comments and suggestions. With the help of a local publishing service our frequently revised draft emerged as a guide in a ring binder format which could be customised for each patient.

Findings : Outcome:
A 64 page guide with a glossary and index which is concise and easy to understand for patients and their families.

Conclusion/Evaluation:
The guide has an evaluation form 91% report the guide has the correct amount of information.

Relevance
Providing appropriate information to predialysis patients is crucial if we are to offer a good service to renal patients and their families. Research highlights the many positive effects improving communication between patients and health profession has to offer, from quicker recovery from illness to genuinely informed consent and increased patient satisfaction.
